Please sign this form with your security deposit and sign forms (after shared with those concerned) from the photographer/caterer/florist to the church office within 30 days of the wedding date. I hereby understand the policies stated above and take full responsibility for the actions of the wedding party and guests. I also understand that the security deposit may not be refunded if deemed necessary by the church staff due to damage to the church facilities during the wedding activities. I also understand that I must return the initialized forms for the caterer, photographer, and/or florist within 30 days of the wedding date or risk forfeiture of the security deposit.
